Why Lakeland Area Hospice?
Lakeland provides a palliative approach to hospice care. Our team of experienced healthcare professionals offers much-needed help to anyone during this difficult time. Lakeland will address physical, emotional, spiritual, and psychosocial needs which are unique to each individual and family. We work closely with your physician to address your specific goals. If you do not have a personal physician one of our team physicians will direct your care. Lakeland Area Hospice services are available in Erie, Crawford, Venango, Warren, and Mercer Counties of Pennsylvania.
What is Hospice?
Hospice is not a place. It is a philosophy of care. Hospice takes a compassionate approach to healthcare when the goal is comfort rather than cure. Respect, dignity, compassion are the goals of hospice while providing medical care that addresses the physical, emotional, and psychosocial goals of both patients and their families. Hospice is available to anyone facing a life-limiting illness.

Who is eligible?
Who pays for Hospice?
An individual is eligible for Hospice as soon as they have been diagnosed with a life-limiting condition. Among the illnesses our patients have had are:
Alzheimer's /Dementia
Liver Disease Cancer
Neurologic Disease
Cardiac disease
Pulmonary Renal Disease
Stroke/Coma HIV/AIDS

Most health insurances, including Medicare, provide full coverage for Hospice care, usually with no co-payments or deductibles.* In addition, most Medicaid and commercial insurance plans also offer excellent coverage for hospice services with few, if any, out-of-pocket expenses.
How does one get involved with Hospice services?
Your doctor or hospital/care facility case manager may call, or you may call. If you or someone from your family calls, we will call your doctor to discuss the illness and verify that Hospice is medically appropriate. The patient actually becomes admitted to Hospice by signing a consent form electing Hospice care.
